Request for Reconsideration of ADA Coordinator Determination

Any current employee, applicant for employment or admission, or visitor who is not satisfied with the ADA Coordinator’s determination of their disability-related request for accommodation(s) may use this form to submit a request for reconsideration. To initiate a request for reconsideration of an accommodation determination, the party requesting the accommodation must submit a Request for Reconsideration Form to the ADA Coordinator within fifteen (15) Business Days from the date of (1) the ADA Coordinator’s determination; or (2) for concerns regarding implementation of approved accommodations, becoming aware of the basis for the request for reconsideration.

Students with concerns about eligibility determinations for disability-related accommodations must use the Accessible DU: Student Disability Services (SDS) Grievances & Accommodations Appeal process.

The ADA Coordinator did not adequately consider information made available during the interactive process;

The ADA Coordinator’s determination includes factual or analytical errors that would change the determination regarding the requested accommodation(s); or 

The ADA Coordinator had a conflict of interest or bias against the party requesting the accommodation that would change the outcome of the interactive process and/or the ADA Coordinator’s determination regarding the requested accommodation(s).

If an alternative accommodation was approved, state why the alternative accommodation is not sufficient to meet your disability-related need.

The decisionmaker will issue a decision regarding the reconsideration within fifteen (15) Business Days of receipt of the reconsideration request from the ADA Coordinator.
